💡 Wiki Overview

Operational Notice: This wiki documents a tiered caregiver care plan framework designed for part-time personal care settings (14–20 hours/week). It ensures safety, dignity, and compliance while supporting client autonomy.

Intended Use

This is a non-medical resource for caregivers. Caregivers must not perform clinical tasks (e.g., injections) unless licensed. This resource does not replace physician orders or the Master Care Plan.

🧩 Tiered Care Overview

The tiered model provides predictable care delivery regardless of staffing levels or client health fluctuations.

Tier 1: Essential Stability

Focuses on basic safety, oxygen monitoring, and environmental stability. Reminders for ADLs and hydration.

Tier 2: Continuity Support

Active assistance with grooming, meal setup, and laundry. Supports emotional regulation and executive function.

Tier 3: Intensive Support

High-level coordination during health flares. Frequent oxygen checks and active trauma-informed de-escalation.

👜 Daily Support Procedures

ADLs & IADLs

Assistance is provided with the client to support independence. Activities include hygiene, dressing, meal organization, and laundry.

🫂 Emotional & ADHD Support

  • Trauma-Informed: Approach interactions with calm communication. Avoid sudden noises and respect personal boundaries.
  • ADHD Strategies: Use visual schedules and break large tasks into single, manageable steps to prevent executive function overload.
  • Anxiety/PTSD: Maintain predictable routines to reduce emotional dysregulation.

💊 Medication Boundaries (Reminders Only)

Caregivers provide verbal reminders only. Caregivers may assist with opening pre-filled organizers but must not sort medication or determine dosages.

🦠 Safety & Infection Control

  • Oxygen Safety: Keep tanks 10 feet away from open flames/heat. Ensure "No Smoking" signs are visible. Tubing must be managed to prevent trips.
  • Respiratory Safety: Avoid perfumes, aerosols, and strong chemical fumes (bleach) due to client COPD sensitivity.
  • Infection Control: Frequent handwashing and disinfecting of common touch-points.

🆘 Emergency & Disaster Plan

URGENT (Oxygen Dependent): Upon power loss, verify backup "E-tanks" immediately.

Power Outage

Verify backup oxygen tanks. Contact the medical supply company if power is expected to be out >4 hours.

Severe Weather (Tornado)

PTSD Protocol: Provide a comfort kit and maintain a low-tone, calm voice.

Move to the designated interior room (bathroom/hallway). Since housing is first-floor, prioritize the most central windowless area.

Evacuation & Go-Bag

Go-Bag is located in the bedroom closet. Includes 3-day medication supply, oxygen accessories, and emergency contacts.

📑 Standards & Documentation

HIPAA & Privacy

  • No last names or DOB in text messages.
  • No client photos on personal devices or social media.
  • Use approved portal channels for documentation.

👥 Roles & Coordination

  • Caregiver (PCA): On-site support and factual documentation.
  • Supervisor: Approves care plans and resolves coordination issues.
  • Medical Providers: Responsible for all diagnosis and treatment orders.

Overlap is avoided through escalation to supervisors, not unilateral decision-making.
